There is a soft rumble, stirring your mind slightly.  You let your head tilt back without effort, without resistance - eyes closed as you exhale slowly through your nose.  A brief moment of anxiety bubbles up; then fades away- you feel weightless- light and lightheaded.  The calm babbling of a stream of water lies somewhere beyond your reach.  A dull throb makes its way into the foreground and you open your mouth to let out something between a groan and a sigh.  You don't want to wake up, though your mind fogs at why.  Your parched lips peel open as you exhale.
 
But instead of letting out a groan, you feel your chest warp in pain.  A cough makes its way up and you choke it out, gasping for breath.
